If you need to change the address, phone number, email address or name related to your membership, please fill out the form below. There is a $25 charge if, and only if, a new certification with the updated information is required.

Once you press ”submit” you will be taken to PayPal for payment – new certificate/ID card only. Payment must be received before your new certificate can be processed. There is no fee to change your address.
